We appreciate your interest in becoming a patient at the Carolina Center. We need to obtain a great deal of information from you in order to provide the best care, and just as importantly, there is much you will need to learn from us.
4 Step Process to Becoming a New Patient at the Carolina Center:

STEP #3:
PATIENTS WITH LYME DISEASE
Only if you believe you may have Lyme Disease, click here for a Lyme-specific questionnaire.
STEP #4:
SUBMIT MEDICAL RECORDS
Have your doctor fax us your health records or email them directly to us here.
ORIENTATION IF NOT REQUIRED TO BECOME A PATIENT. ANY INDIVIDUAL WHO WISHES TO PROCEED CAN START AT STEP #2 – COMPLETE NEW PATIENT REGISTRATION FORMS AND RETURN TO US.
All forms must be completed prior to scheduling an appointment. Once we have received your completed forms along with the necessary records, you will be contacted by the New Patient Coordinator to schedule your appointment. Appointments are scheduled in the order of which the completed forms are received.

As an integrative medical practice, we work with a wide variety of types of patients from those interested in general wellness to very complex patients with debilitating chronic illnesses. Office visit charges are based on both the provider that is seen and the time spent working on behalf of patients, this including preparation time prior to the office visit, direct contact with the patient either in person or by telehealth, and work spent after the visit creating the treatment plan and nutritional supplement recommendations, ordering labs, and prescribing medications, all which take a considerable amount of time. The total time spent processing most new patients is approximately 4 to 5 hours.

Comprehensive New Patient Visits
Comprehensive New Patient Visits are carried out with Amanda Roper, PA as well as Dr. Pittman. The charge for these providers is:
- Amanda Roper, PA – Comprehensive Visit is $998
- John Pittman, MD – Comprehensive Visit is $1296 (20% discount given for Wilmington area new patients)
THIS CHARGE DOES NOT COVER ANY LAB TESTING, TREATMENTS OR SUPPLEMENTS
We offer a discount for patients already associated with our facility which includes those individuals who have been seen for a Thermography Consult or as a Fast Track to Colon Therapy patient. To qualify for this discount, patients must have been undergone consult or Fast Track provider visit within twelve months of Conversion New Patient Visit.
- Amanda Roper, PA – Comprehensive Visit is $826
- John Pittman, MD – Comprehensive Visit is $1073 (20% discount given for Wilmington area new patients)

Conversion
New Patient Visit- The Conversion New Patient visit is available to individuals who have already been seen either as an Expedited Colon Therapy patient or for a Thermography consultation. Patients must have been seen within the previous 12 months to qualify for this discounted initial office visit.
Comprehensive
New Patient Visit- Our Comprehensive visit is our standard the type of appointment recommended for the majority of patients with complex/chronic conditions or wellness patients wanting the most extensive evaluation.
- Because our goal is to provide the most comprehensive care for complex and preventive-minded patients possible, the key element is our encounter is time.
- A significant portion of the visit is devoted to discussion of our plan for investigation and development of a preliminary treatment plan. Your face-to-face visit with our providers typically lasts 2-3 hours.
Colon Hydrotherapy
Expedited New Patient- Individuals with a particular interest in only Colon Hydrotherapy as a treatment to support detoxification and prevention of disease may choose the Expedited Colon Therapy Program.
- This type visit is only appropriate for individuals without major health issues or for those with only mild digestive symptoms. This is not the setting in which complex chronic health issues can be addressed.
- Approximately 12-16 sessions are typically recommended for best results.
- Subsequent Colon Hydrotherapy appointments are $120 per session.
Expedited Colon Therapy Visit
An Expedited Colon Therapy Visit is offered to those individuals who only wish to undergo colon hydrotherapy and do not have any complicating medical conditions.
- Individuals with an interest in only Colon Hydrotherapy as a treatment to support detoxification and prevention of disease may choose the Expedited Colon Therapy Program. This type of visit is only appropriate for individuals without major health issues or for those with only mild digestive symptoms. This is not the setting in which complex chronic health issues can be addressed.
- This visit includes:
- Initial telephone intake with Colon Therapist
- Brief Office Visit with Amanda Roper, PA (in person or telehealth)
- Colon Therapy Teaching
- Initial Colon Therapy Session
- CHARGE for Expedited Colon Therapy including colonic is $376
THIS CHARGE ONLY INCLUDES ONE COLON THERAPY SESSION
Combined Family Visits: We frequently get requests to see multiple members of a family and can provide Combined Comprehensive Family Visits this being limited to two individuals and only available with Amanda Roper, PA. A discount of $50 per person will be given for the combined. Both family members must be present during the entire office visit to qualify for this discount. Combined visits are not available for established patients. Be aware that more advance notice is typically needed in order to schedule combined visits as this encounter is around 5 hours long.
Combined Family Visits
We frequently get requests to see multiple members of a family and can provide Combined Comprehensive Family Visits this being limited to two individuals and only available with Amanda Roper, PA.
